Preventing Readmission: Following Up With Discharged Patients

If your patient was admitted to the hospital, be sure to schedule a follow-up visit within seven days of discharge.

Patient visits with a Primary Care Provider (PCP) can reduce hospital overutilization for issues that a PCP can treat. PCPs help play a critical role in ensuring patients have a safe transition and get the care they need in an outpatient setting.

To make a smooth transition, it is important to have a process to decide what a patient needs. The healthcare provider can follow this process by creating a discharge plan.
