Oakland Southfield Physicians Announces Newly Approved Care Management Trainers

Congratulations to Annaliese Brindley, PharmD and Molly Klaserner, MA RDN for becoming approved care management trainers. Annaliese and Molly are approved to teach the Introduction to Team Based Care and Patient Engagement courses.

The Introduction to Team Based Care course helps the learner to provide the right care, to the right patient, in the right setting and at the right cost, through a dedicated multidisciplinary care team and in collaboration with the patient, effectively managed in a primary care setting. This course is open to all members of the practice to gain foundational knowledge in Team-Based Care. This course is required for new care managers. It can also be taken as a refresher course for established care managers. If taking as a refresher, CE credits are offered.

The goal of the Patient Engagement course is for care team members to learn engagement tools/skills in order to have productive conversations with patients about their health including basic motivational interviewing skills. It is strongly recommended to take the Introduction to Team Based Care training before taking this course. This course provides 8 hours of PDCM continuing education credits, as well as nursing and social work CE hours.
