Michigan Collaborative for Type 2 Diabetes Cohort 2 Enrollment Opportunity

Oakland Southfield Physicians currently has 23 practices participating in Cohort 1. Additional practices can enroll in Cohort 2 beginning May 1st.

The mission of the Michigan Collaborative for Type 2 Diabetes (MCT2D) is to work with primary care physicians to prevent and reverse the progression of type 2 diabetes. MCT2D has three key initiatives participating primary care physicians are being asked to implement. The three initiatives are:

  1. Expand access to and use of continuous glucose monitoring for type 2 diabetes.
  2. Remove barriers to prescribing newer classes of drug treatments for type 2 diabetes, specifically: GLP-1 receptor agonists and SGLT-2 inhibitors.
  3. Support lower carbohydrate diets.

Additional practices are eligible for participation in Cohort 2 if they are OSP PCMH PCP offices with an EMR AND are a Provider Delivered Care Management (PDCM) office with 20+ Total T2 Diabetes Blue Cross Patients, OR OSP PCMH PCP offices with an EMR AND are a Physician Group Incentive Program (PGIP) office with 75+ Total T2 Diabetes Blue Cross Patients. Cohort 2 enrollment begins May 1st and closes July 15th. Participation in MCT2D results in additional 5% VBR if practice meet specified requirements such as, but not limited to, training requirements, regional meetings, case summaries, learning community activities, etc. during the VBR year.
