Post-Discharge Support for Your High-Risk BCBSM Patients

As part of our Oakland Southfield Physicians (OSP) participation in the BCBSM Blueprint for Affordability program, we are pleased to offer additional support to your office in managing post-discharge follow-up care for a subset of your high-risk BCBSM PPO and Medicare Advantage PPO patients.

OSP has assigned a dedicated registered nurse, Beth McQuiston, RN, BSN, to work with your office to help ensure smooth, patient-centered transitions of care for individuals recently discharged from inpatient or skilled nursing facilities.

Beth will proactively notify your office of high-risk discharges and assist with timely post-discharge follow-up, please note that she will not notify your office for MA PPO patients enrolled in the Honest Health Care in Motion program.

Beth will support your office by:
1.    Monitoring inpatient and SNF discharges for high-risk patients.
2.    Calling your office to support smooth care transitions and continuity of care.
3.    Reminding you to schedule timely follow-up visits (within 7- or 14-days post-discharge). If a timely visit is not possible, she will remind you to bill code 1111F (medication reconciliation) with the office visit, which may be completed up to 30 days after discharge.
