Special Supplemental Benefits for the Chronically Ill (SSBCI) are benefits offered to HAP Medicare Advantage members who have one or more qualifying conditions and who: 

• Are at high risk for hospitalization or adverse health outcomes 
• Require intensive care coordination 

Qualifying conditions can be found on the next page. HAP will verify eligibility for these benefits through claims data or provider attestation of the member’s chronic condition.
